for Older Adults

As we age, it sometimes becomes more difficult to navigate the complexities of our situations. Health concerns, coordination of providers, finances, and changes in living circumstances can all create challenges. But you do not have to do it alone.

Our care managers provide compassionate and professional guidance when you are faced with difficult decisions. We explain options and help you and your loved ones plan for now and the future. Whether you are concerned for yourself, an aging parent or loved one, JFS can help explore options for ensuring that you are not alone during these challenges.

Jewish Family Service’s Older Adult Care Management program helps guide individuals and families during transitional steps in the aging process.

The process involves four steps

Step 1

Consultation

A JFS case manager will meet with the individual, either alone or with a family member present, to discuss the current situation and future needs.

Step 2

Assessment

A JFS case manager will meet with the individual to develop an assessment of needs. An assessment includes, but is not limited to, evaluation of health status, physical capability, mental and emotional status, environmental conditions, and social and financial support networks.

Step 3

Creating a Plan

A plan is created based on the assessment, and is a collaborative process with the individual and family. JFS can contact and arrange for appropriate community resources that help meet individual needs.

Step 4

Monitoring

JFS can arrange for regular monitoring based on individual needs and family requests.
