Case Management for Older Adults

Jewish Family Service’s Case Management program helps to guide individuals and families during transitional steps in the aging process. The process involves four steps:

Step 1: Consultation
A JFS case manager will meet either alone or with a family member present, to discuss their current situation and future needs.

Step 2: Assessment
Our in-home assessment includes, but is not limited to, evaluation of health status, physical capability, mental and emotional status, environmental conditions, and social and financial support networks.

Step 3: Creating a Plan
A plan is created based on the assessment, and is a collaborative process with the client and their family. JFS can contact and arrange for appropriate community resources.

Step 4: Monitoring
JFS can arrange for regular monitoring for clients based on client needs and family requests.
